Kata kunci sistem informasi laporan ketercapaian hasil belajar kurikulum 2013 waterfall Sekolah Menengah Atas Negeri (SMAN) Mojoagung merupakan salah satu sekolahan yang menerapkan kurikulum 2013 yang mulai pada tahun ajaran 2013/2014. Mengadaptasi dari Permen 66 Tahun 2013 didalam kurikulum 2013 nilai rapor siswa nantinya lebih informatif dan deskriptif serta memuat informasi penilaian pada aspek pengetahuan ketrampilan serta sikap siswa. Jadi untuk penilaian rapor pada siswa SMAN Mojoagung nantinya tidak hanya langsung menuliskan nilai akhir tetapi juga harus memuat informasi nilai rapor yang lebih informatif dan deskriptif serta juga memuat informasi penilaian dibeberapa aspek yaitu pada aspek pengetahuan siswa aspek ketrampilan siswa serta aspek sikap siswa. Tujuan penelitian ini adalah untuk melakukan pengembangan dan kelayakan Sistem Informasi Laporan Ketercapaian sesuai format 2013 berbasis Web di Sekolah Menengah Atas Negeri Mojoagung. Diharapkan melalui sistem ini dapat membantu pengguna dan dapat meningkatkan efisiensi dalam proses pelaksanaan memberikan laporan ketercapaian hasil belajar siswa. Sistem ini digunakan oleh tenaga admin sistem guru matapelajaran dan guru wali kelas. Sistem Informasi Laporan Ketercapaian mengadaptasi model pengembangan waterfall yang dikemukakan oleh Pressman dengan tahapan pengembangan antara lain (1) spesifikasi kebutuhan pelanggan (communication) (2) perencanaan (planning) (3) pemodelan (modeling) (4) konstruksi (construction) dan (5) penyebaran (deployment). Pengujian dilakukan pada aspek fungsionalitas dan usabilitas. Pengujian fungsionalitas menggunakan teknik pengujian black-box. Black-box ini dilakukan setelah sebelumnya dilakukan pengujian white-box pengujian white-box dilakukan secara intern. Sedangkan pengujian aspek usabilitas menggunakan skala Likert. Berdasarkan hasil validasi dan pengujian sistem didapatkan persentase rata-rata skor usabilitas sistem sebesar 94 62%. Sedangkan hasil validasi dan uji coba aspek fungsionalitas sistem sebesar 92 8% yang berarti bahwa semua validasi dan uji coba tersebut menyatakan bahwa Sistem Informasi Laporan Ketercapaian valid dan layak digunakan. Adapting from Rule of Ministry 66 In 2013 in the curriculum 2013 student s report scores will appear more informative and descriptive as well contain the information of students assessment of cognitive phsycomotor and affective aspects. Therefore the assessment report on Mojoagung high school students will not only directly appearing the final scores but also loading the information of report scores which are more informative and descriptive as well contain the assessment informations there are cognitive phsycomotor and affective aspects too. The purpose of this research is to develop the achievement study of report information system web-based according to curriculum 2013 and determine its feasibility. Hopefully this system can help users and improve the efficiency of achievement study of student report. This system is used by admin system staff subject/classroom teachers and counseling teachers. Attainment report information system adapt waterfall development model that proposed by Pressman with development phases (1) communication (2) planning (3) modeling (4) construction and (5) deployment. Tests carried out on usability and functionality aspects. Functionality aspect was tested using black-box techniques. Black-box testing was performed after white-box testing in this case white-box testing was performed internally. Meanwhile the usability aspects was tested using Likert scale. The validation and testing result shows average percentage score of usability system equals to 94 62%. Meanwhile functionality test result shows average percentage score equals to 92 8%. It means that all system functionality works as expected. This result certify that the system is valid and feasible to use.
