Kata Kunci kemampuan membayar kemauan membayar Jaminan Kesehatan Nasional Program Jaminan Kesehatan Nasional (JKN) bertujuan untuk mewujudkan ekuitas pelayanan kesehatan bagi masyarakat Indonesia. Tahun 2019 ditargetkan program JKN telah mencapai cakupan semesta (universal coverage). Hingga akhir Desember 2014 jumlah peserta JKN mencapai 133 4 juta jiwa atau 52 9% dari total penduduk Indonesia. Jumlah penduduk belum tercakup JKN di Jawa Timur mencapai 50 2% dan menjadikan Jawa Timur sebagai provinsi peringkat kedua jumlah penduduk terbesar belum tercakup JKN. Kota Malang yang merupakan bagian dari provinsi Jawa Timur memiliki jumlah penduduk tercakup JKN sebesar 67 71% dengan rata-rata cakupan peserta per kecamatan sebesar 64% pada tahun 2016. Kelurahan Sawojajar menjadi bagian dari Kecamatan Kedungkandang yang merupakan wilayah dengan jumlah penduduk terbesar belum tercakup JKN. Masyarakat enggan menjadi peserta JKN dengan alasan tidak memiliki kemampuan untuk membayar iuran JKN terlebih dengan pemberlakuan pendaftaran untuk seluruh anggota keluarga. Tujuan dari penelitian adalah untuk menghitung kemampuan membayar dan kemauan membayar masyarakat terhadap Jaminan Kesehatan Nasional (JKN). Identifikasi mengenai kemampuan dan kemauan membayar penduduk dapat digunakan sebagai dasar menentukan kebijakan strategi dan program tepat guna yang bertujuan untuk meningkatkan cakupan peserta JKN. Penelitian dilaksanakan dengan pendekatan deskriptif kuantitatif. Variabel penelitian berupa pendapatan pengeluaran pangan dan pengeluaran non pangan rumah tangga yang diukur secara potong lintang. Data penelitian dikumpulkan melalui wawancara kepada 120 responden. Responden dipilih dengan cara accidental sampling. Metode household budget dan stated preference berturut-turut digunakan untuk mengukur kemampuan dan kemauan membayar. Hasil penelitian menunjukkan rata-rata ability to pay (ATP) responden Rp34.404 00. Sebesar 44 2% responden berada pada kategori ATP Rp25.500 00 dan 76 7% responden diketahui memiliki kemauan membayar iuran JKN. Hasil penelitian menunjukkan persentase WTP lebih besar dari ATP sehingga dapat disimpulkan kemauan membayar masyarakat lebih besar dari kemampuan membayar iuran JKN. Stakeholder terkait perlu menggalakkan sosialisasi dan bantuan proses pendaftaran untuk memfasilitasi penduduk yang ingin menjadi peserta JKN. Keywords ability to pay willingness to pay National Health Insurance The purpose of National Health Insurance (NHI) program is to succeed equality on healthcare. In 2019 it is targeted that the JKN program has reached universal coverage. Until the end of 2014 the nimber of participants reached 133 4 million people or 52 9% of teh total population of Indonesia. The number of residents not covered by JKN in East Java has reached 50 2% and maked East Java as the second largest number of population not covered by JKN. Malang City which is part of East Java province has 67 71% JKN participants with an average 64% for each subdistrict. Sawojajar is part of Kedungkandang which is the subdistrict with the largest population not yet covered by JKN. People are refuse to become JKN participants by reason of not having ability to pay JKN dues especially with enrollment for all family members. The aims of this study is to measure the ability and willingness to pay of Sawojajar National Health Insurance (NHI) residents. Identification of the ability and willingness to pay can be used as a basis for determinign appropriate policies strategies and programs aimed at increasing the scope of JKN participants. This research used descriptive quantitative approach. The research variables are household income food expenditure non food expenditure ability and willlingness to pay which was measured with cross sectional method. Interview were conducted to 120 respondents. Subject were drawn by accidental sampling. Household budget method and stated preference method are used to measure the ability and willingness respectively. The result found that the average of ability to pay (ATP) is Rp34.404 00. As many as 44 2% respondent s ability to pay (ATP) is Rp25.500 00 and 76 7% of respondents are known to have willingness to pay (ATP) JKN premium. The results showed that the percentage of WTP is greater than ATP. The research pointed that Sawojajar residents are mostly will to pay the pemium but not able to pay the cost. Relevant stakeholders need to promote socialization and assistance of the registration process to facilitate the people who want to become JKN participants. The goverment needs to improve health financing system in order to achieve universal coverage goal.
